In a police station in a small village, a sergeant interrogates a young and frightened postman, known as "Cacasotto". He, normally known for being calm and good-hearted, was arrested for having threatened a policeman on patrol with a knife. Eventually, Cacasotto begins to speak with the cop. And he tells what happened twenty years earlier to him and to his five best friends: Gianluca, Walter, Peppino, Margherita and his little brother, Andrea.

The plot of the film unfolds over 24 hours during which everything happens, including a robbery. A Californian tourist, Sophie, played by Elena Kampouris, meets three young people on the beach who promise to introduce her to the Sicilian capital, but actually have a debt to pay and involve the girl. The three young men are played by Lorenzo Richelmy, Enrico Inserra and Saul Nanni.

After several mysterious accidents, the game is interrupted and the players leave the bunker while the staff remain behind to investigate the disappearance of Greg, the mastermind of the game. They soon find themselves trapped inside and in peril as they begin to die in mysterious ways and the group realizes that someone or something paranormal is playing a twisted game with them which quickly plunges into a terrifying fight for survival.

Dj Steph is a young radio deejay on the rise, who gained a lot of popularity on social media and every evening hosts a radio show with a large following, during which he receives calls from his fans. One evening, a call chills him to the bone: a cold-blooded stranger announces on live radio that he plans to take his own life, making himself explode in the middle of the city.
